A 1.5-inch sample containing about 200 hair strands (equivalent to the width of a #2 pencil) closest to the scalp yields 100mg of hair, which is ideal for screening and confirmation tests. For add-ons like EtG and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair cannot be collected, an equal amount of body hair can be used. When discussing head hair, we mean scalp hair specifically. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The process of handling a drug test result in the lab involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, involving the sample's intake into a lab's system. It includes verifying the sample's seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional required data entry not covered by an electronic chain of custody.

Screening entails a preliminary check for drugs of abuse. While cost-effective for ruling out drug usage on most samples, a positive screen must be confirmed to be valid in legal contexts. Samples that are possibly positive during Screening need further confirmation.

If a sample appears positive during Screening, additional hair is sourced from the initial specimen and readied for Extraction. Drugs are extracted from hair at more reduced concentrations compared to other methods (like urine or oral fluid), making hair drug screening the most challenging method to conduct.

Confirmation of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleaned before confirmation if required. The complete lab process from Accessioning to Confirmation is reviewed according to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
