What it is, How it works

Hair analysis has emerged as a significant method for identifying drug and alcohol consumption. Hair offers an extended history of alcohol and other substances by capturing biomarkers within the fibers of the developing hair strand. When gathered near the scalp, hair can deliver a detection window extending up to roughly three months for alcohol and various drugs. Hair is straightforward to collect, comparatively challenging to tamper with, and straightforward to transport.

A 1.5-inch section of roughly 200 hair strands (approximately the width of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, supplementary tests, and/or panels beyond 10, a specimen of 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be gathered. When mentioning head hair, we mean scalp hair exclusively. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory process for determining a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a laboratory’s system. This encompasses ensuring the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and finalizing any further data input missing from an electronic chain of custody system.

Screening encompasses a rapid preliminary assessment for substances of abuse. Whereas Screening is a cost-effective means to exclude drug usage in most samples, a positive screen must be verified to hold up in court. Samples that are potentially positive in Screening must undergo secondary confirmation.

If a specimen is potentially positive at the Screening stage, additional hair is extracted from the primary sample and readied for Extraction. During this phase, drugs are extracted from hair at significantly reduced concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging methodology to execute.

Verification of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive are cleansed before confirmation as necessary. The complete laboratory workflow from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
