The laboratory processing of a drug test result involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample into a laboratory's database. When a sample is presumed positive in the Screening step, additional hair is extracted from the initial sample and set up for Extraction. During this phase, substances are extracted from hair at a significantly lower concentration than within other methods (such as urine or oral fluid), which makes hair drug testing one of the most complex procedures to conduct.

Positive screening outcomes are verified through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity are cleaned prior to confirmation as required. The entire laboratory process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and follows the ISO / IEC 17025 accreditation standards.
