What it is, How it works

The use of hair testing is increasingly recognized as a robust method for uncovering drug and alcohol consumption. Hair essentially records a long-term use history by ensnaring biomarkers within the structure of the hair strand as it grows. If collected near the scalp, hair can provide insight into drug and alcohol use over a period of up to about three months. Hair collection is straightforward, not easily tampered with, and convenient to send.

A hair sample of 1.5 inches comprising roughly 200 hair strands (comparable to the thickness of a #2 pencil) taken nearest the scalp delivers 100mg of hair, the preferred amount for assessment and verification. For EtG, additional tests, or screenings beyond 10 panels, a specimen quantity of 150mg is advised. It is suggested to weigh the specimen using a jeweler's scale. If scalp hair cannot be obtained, a similar weight of body hair can be used instead. "Head hair" specifically refers to scalp hair, whereas "body hair" pertains to any other hair varieties (facial, underarm, etc.).

Process Overview

There are four primary phases in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial onboarding of a sample into the lab's system. This step includes confirming that the sample was securely sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional entries not covered by an electronic chain of custody system.

Screening entails a preliminary swift analysis for drug abuse markers. Although Screening is a cost-efficient technique for eliminating drug use in most samples, any positive findings in Screening must undergo confirmation to be court-admissible. Samples that yield presumptive positive results in Screening necessitate further verification.

When a sample flags as presumptively positive during Screening, additional hair is extracted from the initial sample for the Extraction phase. In this step, drugs are extracted from hair at much lower concentrations compared to other methods (such as urine or oral fluids), establishing hair drug testing as the more challenging methodology to execute.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positive results are cleansed before Confirmation, as needed. The complete laboratory process from Accessioning to Confirmation undergoes review in line with both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
