Accessioning consists of the initial handling of a sample into a lab’s system. This encompasses validating the sample’s proper sealing and shipment, allocating a random laboratory accessioning number (LAN), and completing any required data entry absent in an electronic custody chain system.
Screening entails an initial swift check for substance abuse. While it remains a cost-effective approach for ruling out substance use in the majority of samples, a positive screening requires confirmation to be legally acceptable. Any samples preliminarily positive in Screening necessitate secondary confirmation.
If a sample returns presumptively positive during Screening, additional hair is retrieved from the initial specimen for Extraction. In this stage, substances are extracted from hair at a significantly reduced concentration than other methods (e.g., urine or oral fluid), making hair drug testing a challenging methodology.
Confirming any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The comprehensive laboratory process from Accessioning to Confirmation is examined per both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
