Extracting a 1.5-inch section of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg, which is the optimal quantity for initial and confirmatory testing. For EtG analyses, supplementary diagnostics, and/or panels exceeding 10, a 150mg sample is advisable. We suggest the specimen is weighed using a jeweler’s scale. In the absence of scalp hair, a proportional quantity of body hair may be utilized. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other types, such as facial or axillary hair.

Process Overview

The laboratory processing of drug test results involves four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s system. This process includes validating that the sample was appropriately sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent in an electronic chain of custody system.

Screening delivers an expedient preliminary assessment for substances of abuse. While Screening affords a cost-effective approach to negate drug consumption in most samples, a positive screen mandates confirmation to be legally defensible. Samples indicating a presumptive positive in Screening necessitate subsequent verification.

Should the Screening phase yield a presumptive positive, additional hair is withdrawn from the original sample for the Extraction phase. In this process, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), rendering hair drug tests the most challenging to administer.

Confirmatory analysis of any positive screen result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed pre-confirmation as appropriate. The entire lab procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
