What it is, How it works

Hair captures a long-term record of alcohol and drug presence by retaining biomarkers within the structure of the growing hair fiber. When taken close to the scalp, hair can offer up to a 3-month detection timeframe for alcohol and drugs. Hair is easy to acquire, somewhat resistant to tampering, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, an optimal sample size for screening and validation. For EtG, additional tests, and/or panels beyond 10, a specimen of 150mg is advised. We suggest measuring the specimen with a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be used. When mentioning head hair, we specify scalp hair exclusively. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The four key steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample into the lab’s system. This step involves ensuring the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening consists of a preliminary rapid test for drugs of abuse. While Screening is an economical method to exclude drug consumption for most samples, a positive result must be confirmed for court acceptance. Samples indicating positive in Screening require further confirmation.

If a sample indicates a presumptive positive during Screening, additional hair is taken from the original specimen and readied for Extraction. At this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug testing is the most intricate methodology to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleaned before confirmation as necessary. The full lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair standard and accreditation to ISO / IEC 17025 criteria.
