What it is, How it works

If gathered near the scalp, hair can present nearly a 3-month timeframe for detecting alcohol and drugs. It's straightforward to gather, relatively resistant to tampering, and easily transportable.

A 1.5-inch specimen of roughly 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for assessment and reaffirmation. For EtG, extras, and/or examinations exceeding 10 panels, 150mg of the specimen is advised. We propose measuring the specimen on a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair may be acquired. When discussing head hair, we mean scalp hair exclusively. Body hair denotes all other hair varieties (facial, axillary, etc.).

Process Overview

The primary stages entailed in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into a laboratory’s framework. This involves confirming the sample was sealed and dispatched accurately, allotting a random LAN (Laboratory Accessioning Number), and finishing any additional data entry not available through an electronic chain of custody system.

Screening constitutes an initial rapid examination for substance abuse. While Screening is a budget-friendly approach to dismiss drug usage on most samples, a positive screen must be confirmed to be admissible in legal settings. Any samples that are tentatively positive in Screening necessitate an additional confirmation.

If a sample appears tentatively positive in the Screening stage, more hair is extracted from the initial sample and readied for Extraction. In this stage, drugs are drawn from hair at much lower concentrations than in other methodologies (ex. urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as needed. The entire laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
