What it is, How it works

When obtained close to the head, hair can offer nearly a 3-month detection window for alcohol and drugs. Hair is easy to gather, fairly challenging to tamper with, and straightforward to transport.

A 1.5-inch section consisting of about 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the ideal sample for both screening and confirmation. For EtG, additional tests, and/or evaluations exceeding 10 panels, 150mg of the sample is advised. It's suggested to measure the sample on a jeweler’s scale. If scalp hair is not an option, the same quantity of body hair can be collected. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (such as facial, axillary hair, etc.).

Process Overview

There are four primary steps involved in the laboratory processing of a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample into the lab’s system. This process involves verifying the sample’s proper sealing and shipping, allocating a random LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid test for drugs of abuse. Although Screening is an economical way to exclude drug usage in most samples, a positive result requires confirmation to be valid in legal settings. All samples that test presumptively positive during Screening must undergo a secondary confirmation.

If a specimen tests presumptively positive at the Screening stage, additional hair is extracted from the original sample for Extraction. During this phase, drugs are drawn from hair at a significantly lower concentration than in other methodologies (for example, urine or oral fluid), which makes hair drug testing the most challenging methodology to implement.

Confirmation of any positive screening results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if required. From Accessioning to Confirmation, the whole laboratory procedure is conducted under the oversight of the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
