What it is, How it works

Hair captures a history of alcohol and other substances by embedding biomarkers within the fibers of the developing hair strand. When taken from near the scalp, hair can offer up to a roughly 3-month timeframe for detecting alcohol and other substances. Hair is easy to gather, relatively immune to tampering, and simple to transport.

A sample measuring 1.5 inches consisting of about 200 strands of hair (approximately the size of a #2 pencil) next to the scalp will yield 100mg of hair, the ideal quantity for analyzing and verifying. For EtG, add-ons, and/or screenings exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample using a jeweler’s scale. Should scalp hair be unavailable, an equivalent volume of body hair may be used. Head hair specifically refers to scalp hair. Body hair includes all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test follows four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the early processing of a specimen into a laboratory’s records. This process checks that the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any further data input not included by an electronic chain of custody system.

Screening offers an initial rapid assessment for substance misuse. Although Screening is an economical method to dismiss drug usage in most specimens, a positive screen requires confirmation to be valid in legal contexts. Specimens that appear positive in Screening need additional confirmation.

Should a specimen appear positive during the Screening stage, more hair is drawn from the initial sample and readied for Extraction. During this process, substances are extracted from hair at a much lower concentration than in other techniques (such as urine or oral fluid), making hair drug screening the most challenging technique to execute.

Confirmation for any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All ostensibly positive samples are cleansed prior to confirmation if required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
