Gathering a 1.5-inch piece of around 200 hair strands (comparable to a #2 pencil thickness) nearest to the scalp results in 100mg of hair, which is the optimal sample size for screening and verification. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the sample is advised. It is advised to measure the sample using a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair may be taken. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the phase of entering a sample into the laboratory’s system. This involves confirming that the sample was properly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry typically supplied by an electronic chain of custody system.

Screening includes an initial speed check for abuse drugs. Though Screening is cost-efficient for most samples, a positive screen demands confirmation to be admissible legally. Any samples that are presumptively positive during Screening require additional confirmation.

In cases where a sample is presumptively positive during the Screening phase, more hair is extracted from the initial sample and readied for Extraction. At this stage, drugs are removed from hair at much lower concentrations than with other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
