What it is, How it works

Hair evaluations have gained recognition as a significant means for identifying drug and alcohol consumption. Hair retains a prolonged record of alcohol and drug usage by embedding biomarkers within the hair strand's fibers as it grows. When the hair is taken near the scalp, it can reveal up to around a 90-day timeframe for detecting alcohol and drugs. Collecting hair is straightforward, somewhat resistant to tampering, and convenient to transport.

A sample of 1.5 inches comprising roughly 200 hair strands (similar in size to a #2 pencil) closest to the scalp yields 100mg of hair, which is the perfect sample size for both screening and verification. For EtG, supplemental tests, and/or panels exceeding 10, a 150mg sample of the specimen is advisable. It’s suggested to measure the specimen using a jeweler’s scale. If scalp hair isn't available, body hair of equal quantity can be utilized. References to head hair specifically mean scalp hair. Body hair includes all other hair types (e.g., facial, axillary).

Process Overview

The processing of a drug test result in the laboratory involves four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the preliminary handling of a sample into the lab’s system. This step checks the sample for proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and finalizes any remaining data not included by an electronic chain of custody system.

Screening is an initial quick analysis for drug abuse substances. Although Screening is a cost-effective approach to exclude drug use in most cases, a positive result must be validated to be permissible in legal settings. Samples deemed presumptively positive during Screening necessitate a secondary Confirmation.

For samples preliminarily positive in Screening, additional hair is extracted from the original sample for Examination. Drugs are isolated from hair at notably lower concentrations than other methods (e.g., urine or oral fluids), making hair drug screening the most challenging procedure.

Confirmation of a positive screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All provisional positive samples undergo a cleaning process before confirmation as required. The entire examination process from Accessioning through to Confirmation is evaluated under the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
