What it is, How it works

Hair analysis has emerged as a robust method for identifying drug and alcohol exposure. Hair retains a long-term record of substance use by capturing biomarkers within the fibers of the growing strand. When taken close to the scalp, hair can offer up to a three-month window for detecting alcohol and drugs. Hair is easy to gather, challenging to tamper with, and simple to transport.

A 1.5-inch sample containing about 200 strands (about the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal quantity for analysis and confirmation. For tests such as EtG add-ons and/or those over 10 panels, 150mg of the sample is advised. We suggest the use of a jeweler’s scale to measure the specimen weight. If scalp hair is not present, an equivalent amount of body hair can be used. When we mention head hair, we specifically refer to scalp hair only. All other hair types (like facial or axillary hair) fall into the category of body hair.

Process Overview

There are four essential steps in the lab processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a lab's system. This includes checking that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry not available through an electronic chain of custody system.

Screening involves a preliminary check for substance abuse. Although Screening offers a cost-efficient way to exclude drug use in the bulk of samples, a positive screen must be corroborated for it to be valid in legal proceedings. Any presumptively positive samples in Screening require secondary confirmation.

If Screening shows a presumptive positive result, additional hair is taken from the primary specimen for Preparation in Extraction. Here, substances are extracted from hair at a significantly lower concentration than in other methods (such as urine or oral fluid), making hair drug testing the most intricate technique to perform.

Confirmation of any positive screening results is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged as presumptively positive are cleansed prior to confirmation if required. The entire lab procedure, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair classification and conforms to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
