A hair sample of 1.5 inches, consisting of around 200 strands of hair (the diameter of a #2 pencil) nearest to the scalp, yields 100mg of hair, which is ideal for screening and confirmation. For EtG, additional testing, or tests exceeding 10 panels, a 150mg sample is suggested. We recommend using a jeweler's scale for weighing the specimen. If scalp hair is not available, an equivalent amount of body hair may be used. When discussing head hair, we mean scalp hair exclusively. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into the lab's system. This includes verifying the proper sealing and shipment of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any additional data entry lacking from an electronic chain of custody system.

Screening is an initial swift assessment for drugs of abuse. Although Screening is cost-efficient for excluding drug use in most samples, a positive screening result requires confirmation for legal validity. Any samples that show presumptive positivity in Screening need subsequent confirmation.

If a sample shows presumptive positivity during Screening, additional hair is taken from the original sample and readied for Extraction. In this phase, drugs are extracted from hair at lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation of any positive screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples are cleaned before confirmation if necessary. The complete process from Accessioning to Confirmation is conducted under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
