What it is, How it works

Hair tests are now recognized as a robust method for detecting drug and alcohol use. Hair encapsulates a lengthy history of drugs and alcohol by embedding biomarkers within the strands as they grow. When cut near the scalp, hair can offer roughly a three-month detection timeframe for alcohol and other substances. Collecting hair is straightforward, hard to tamper with, and can be easily mailed.

A 1.5-inch sample made up of about 200 strands of hair (roughly the diameter of a #2 pencil) nearest to the scalp yields 100mg, the optimal amount for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is suggested. It is advisable to weigh the sample using a jeweler’s scale. Should scalp hair be unavailable, an equivalent portion of body hair can be collected. When we mention head hair, we refer specifically to scalp hair. Body hair encompasses all other types (like facial, axillary, etc.).

Process Overview

The process of lab analysis for a drug test outcome comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial incorporation of a specimen into the lab’s system. This phase includes confirming the sample's seal and proper shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entries absent from an electronic chain of custody system.

Screening represents a preliminary examination for substance abuse. Although Screening serves as a cost-effective method to rule out drug use in most cases, a positive result necessitates confirmation to hold up in court. Any samples that show presumptive positives during Screening require further verification.

If a preliminary positive is noted in the Screening phase, additional hair from the original specimen is used for Extraction preparation. During Extraction, drugs are isolated from hair at much lower concentrations than in other techniques (such as urine or oral fluids), making hair drug testing among the most challenging methodologies.

Confirmation of any positive result from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo a washing process before confirmation when necessary. The entire lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair standard and accredited to ISO / IEC 17025 criteria.
