What it is, How it works

Hair analysis is perceived as a robust method for identifying substance and alcohol consumption. Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of the elongating hair shaft. When harvested near the scalp, hair can offer up to a roughly 3-month period for detecting alcohol and other substances. Hair is straightforward to gather, relatively resistant to tampering, and simple to transport.

A 1.5-inch fragment consisting of roughly 200 hairs (approximately the size of a #2 pencil) closest to the scalp will provide 100mg of hair, which is the ideal sample for testing and verification. For EtG, and/or analyses beyond 10 panels, 150mg of the sample is suggested. We suggest weighing the sample on a jeweler’s scale. If scalp hair isn’t accessible, a comparable quantity of body hair can be retrieved. When discussing head hair, we mean scalp hair only. Body hair refers to every other type of hair (facial, axillary, etc.).

Process Overview

The four primary stages in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial entry of a sample into a lab’s system. This step involves confirming that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any other data input not covered by an electronic chain of custody process.

Screening consists of a preliminary rapid review for substances of abuse. While Screening is an economical strategy to eliminate drug use on most samples, a positive screen must be authenticated to be valid in court. Any samples that show a presumptive positive are required to undergo further validation.

If a sample indicates a presumptive positive in the Screening phase, additional hair is retrieved from the initial sample and readied for Extraction. During this phase, substances are extracted from hair at significantly lower levels than other techniques (for instance, urine or oral fluid), which is why hair drug testing is the most intricate method to execute.

Authentication of any positive screen outcome is done through GC/MS, GC/MS/MS, or LC/MS/MS. Samples presumed positive are washed before confirmation if necessary. The entire laboratory sequence from Accessioning to Confirmation is examined within the CAP (College of American Pathologists) Hair classification and the accredited ISO / IEC 17025 standards.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
