For EtG, additional tests, and/or panels exceeding ten, a 150mg sample size is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair is not accessible, a comparable volume of body hair can be used. When mentioning head hair, it specifically refers to scalp hair. Body hair includes all other types (like facial and axillary hair).

Process Overview

The laboratory processing of a drug test result includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning handles the initial incorporation of a sample into a lab system. This step entails validating that the sample was sealed and delivered accurately, generating a unique LAN (Laboratory Accessioning Number), and completing any ancillary data not accessible via an electronic custody chain system.

Screening is a preliminary swift assessment for drugs of misuse. Though Screening is a budget-friendly option to dismiss drug use in most cases, a positive screen must be verified for court acceptance. Samples presumed positive during Screening necessitate a follow-up confirmation.

For samples presumed positive during Screening, additional hair is taken from the original sample for Extraction. In this stage, drugs are retrieved from hair at a concentration significantly lower than other methods (e.g., urine or oral fluid), thus making hair drug screening the most arduous method to conduct.

Any positive screening result undergoes GC/MS, GC/MS/MS, or LC/MS/MS for Confirmation. All samples presumed positive are cleansed before confirmation as warranted. The complete lab operation from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair distinction and in line with ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
