What it is, How it works

Hair captures a long-term record of alcohol and other drugs by embedding biomarkers within the strands as they grow. When collected from near the scalp, hair can offer a detection window of up to roughly three months for alcohol and other drugs. Hair is straightforward to collect, relatively tough to tamper with, and uncomplicated to transport.

A 1.5-inch segment of approximately 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the perfect sample for both screening and confirmation purposes. For EtG, additions, and/or tests exceeding 10 panels, it is suggested to use 150mg of the sample. We advocate for weighing the sample using a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair can be used. When mentioning head hair, we specifically refer to scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test consists of four essential phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into the laboratory’s framework. This phase includes confirming that the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not processed by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is an economical means to eliminate drug usage in most specimens, a positive result requires confirmation to be valid in legal settings. Samples that appear positive during Screening necessitate further confirmation.

In cases where a sample is presumably positive in the Screening phase, additional hair from the initial sample is utilized for Extraction. During this step, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), which makes hair drug analysis the most challenging method to perform.

Confirmation of any positive Screening result is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation when needed. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
