What it is, How it works

When hair is sampled near the scalp, it can provide an alcohol and drug-use detection window of up to approximately three months. Hair collection is straightforward, relatively difficult to tamper with, and convenient for shipping.

Collecting a 1.5-inch sample of around 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, supplementary tests, and/or panels exceeding 10, it is recommended to collect 150mg of the sample. It's advisable to weigh the sample using a jeweler's scale. If scalp hair is unattainable, an equivalent quantity of body hair may be used. "Head hair" specifically refers to scalp hair, while "body hair" encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The primary stages in the laboratory analysis of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step where a sample is initially processed within a laboratory's system. This stage includes ensuring the sample was sealed and conveyed correctly, generating a random LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-efficient approach to eliminate drug use concerns for most samples, any positive result from screening requires confirmation to be considered valid in a legal context. Samples that give a presumptive positive in Screening necessitate a secondary confirmation.

When a sample shows a presumptive positive result during Screening, more hair is extracted from the initial batch and processed for Extraction. During Extraction, drugs are drawn from hair at a more diluted concentration compared to other processes (such as urine or oral fluid), making hair drug screening the most complex method to execute.

Positive screening outcomes are verified through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing prior to confirmation when necessary. The complete laboratory routine from Accessioning to Confirmation is assessed following the guidelines of the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
