A 1.5-inch section consisting of about 200 hair strands (akin to the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is the optimal sample for screening and validation. For EtG assessments, additional tests, and/or panels exceeding 10, 150mg of sample is advisable. We suggest weighing the hair using a jeweler’s scale. If scalp hair isn't available, an equal quantity of body hair can be taken. By "head hair," we mean scalp hair exclusively. "Body hair" encompasses all other forms of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into the lab's system. This step involves ensuring that the sample is adequately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry not supported by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. While Screening helps to efficiently eliminate drug consumption in most samples, a positive screen must be validated for legal admissibility. Any sample that shows a presumptive positive in Screening must undergo secondary confirmation.

If a sample is presumptive positive during Screening, additional hair from the initial sample is utilized for Extraction. This phase involves extracting drugs from hair at a considerably lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing presumptive positives are washed before confirmation as necessary. The whole laboratory workflow from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
