Hair collection is straightforward, somewhat challenging to tamper with, and easy to transport.

A hair sample measuring 1.5 inches, encompassing about 200 hairs (comparable to a #2 pencil’s thickness) near the scalp offers 100mg of hair, which is optimal for analysis and confirmation. For EtG, additional testing beyond 10 panels, 150mg of hair is advisable. We suggest weighing the hair on a jeweler’s scale. Should scalp hair be unattainable, a similar amount of body hair can be taken. Head hair pertains exclusively to scalp hair whereas body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The pivotal steps in laboratory testing for drug detection are Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary processing of a sample into the lab’s system. This ensures the sample was adequately sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and concludes any extra data entry absent from an electronic chain of custody system.

Screening conducts an initial rapid assessment for substances of abuse. Although cost-effective for excluding drug use in the majority of samples, a favorable screen necessitates confirmation for legal validity. Samples hinting positivity in Screening need further confirmation.

In cases of presumptive positivity during Screening, more hair is taken from the initial sample and readied for Extraction. Here, substances are retrieved from hair at significantly low concentrations unlike other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Any favorable screening findings undergo Confirmation through GC/MS, GC/MS/MS or LC/MS/MS. All suspected positive samples are cleansed before confirmation as required. The complete lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
