What it is, How it works

Hair screening is now recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of substances by retaining biomarkers within the strands of hair as they grow. When sourced near the scalp, hair can present nearly a 3-month detection timeframe for drugs and alcohol. Collecting hair is straightforward, relatively tamper-resistant, and easy to transport.

A sample measuring 1.5 inches, containing about 200 hairs (similar in width to a #2 pencil) nearest the scalp, produces 100mg of hair, the optimal amount for analysis and verification. For EtG assessments, additional components, and/or tests exceeding 10 panels, it is advised to use 150mg of the material. We suggest measuring the sample using a jeweler’s scale. In the absence of scalp hair, a comparable quantity of body hair may be utilized. When mentioning head hair, we refer solely to scalp hair. Body hair comprises all other types (facial, axillary, etc.).

Process Overview

The pivotal procedures in the lab processing of a drug test outcome encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary processing of a specimen within a lab’s framework. This process involves ensuring the sample is properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic custody documentation system.

Screening constitutes an initial rapid examination for drugs of abuse. Though Screening offers an economical solution to exclude drug usage in most samples, a positive result necessitates verification for legal acceptability. Samples showing tentative positivity during Screening need subsequent confirmation.

Upon preliminary positivity within the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, substances are retrieved from hair at a substantially lower concentration compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive results in Screening occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All samples initially deemed positive undergo washing before confirmation as required. The comprehensive lab process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
