For EtG, extra tests, and/or assessments beyond 10 panels, 150mg of the sample is advisable. Using a jeweler’s scale is suggested for weighing the sample. In situations where scalp hair is absent, a comparable quantity of body hair can be utilized. The term head hair refers solely to scalp hair. In contrast, body hair encompasses all other hair types (such as facial and axillary).

Process Overview

The principal stages in processing a drug test result at the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial assimilation of a sample into a lab's system. This step involves confirming that the sample was appropriately sealed and delivered, allocating a random LAN (Laboratory Accessioning Number), and executing any supplementary data entry not offered by an electronic chain of custody mechanism.

Screening constitutes an initial rapid assessment for drug abuse. Screening is a cost-effective approach to exclude drug use in the majority of samples; however, a positive screen necessitates confirmation to be valid in a legal setting. Samples that are presumptively positive in Screening indeed require subsequent verification.

When a sample is tentatively positive during Screening, more hair is extracted from the primary sample and set for Extraction. During Extraction, drugs are isolated from hair at a significantly lower concentration compared to other methodologies like urine or oral fluid, attributing to the complexity of performing hair drug testing.

Confirmation of any positive screen is done using GC/MS, GC/MS/MS, or LC/MS/MS. Every tentative positive sample undergoes a wash before confirmation as necessary. The entire lab procedure, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and is accredited according to ISO / IEC 17025 standards.
