A 1.5-inch sample consisting of around 200 hair strands (comparable to a #2 pencil in thickness) closest to the scalp will yield 100mg of hair, the optimal amount for analysis and verification. For EtG, add-ons, and/or tests over 10 panels, 150mg of the sample is advisable. It's best to weigh the sample using a jeweler’s scale. If scalp hair is not available, a similar volume of body hair can be collected. Referring to head hair means exclusively scalp hair. Body hair encompasses all other types like facial, axillary, etc.

Process Overview

The drug test outcome processing in a lab involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into a lab’s system. It involves confirming that the sample was properly sealed and sent, issuing a random LAN (Laboratory Accessioning Number), and entering any further data absent from an electronic chain of custody system.

Screening is a preliminary rapid examination for prohibited substances. While Screening is an economical method to exclude drug use in most cases, a positive screen must be confirmed for legal credibility. Any samples that presumptively test positive during Screening require secondary verification.

If a sample tests presumptively positive in the Screening phase, additional hair is extracted from the original sample for the Extraction step. Here, drugs are obtained from hair in significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), which makes hair drug screening the most complex technique to execute.

Confirmation of any positive screening result utilizes GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleaned before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is reviewed in line with both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
