For EtG, additional tests, and/or panels beyond 10, it is advisable to obtain 150mg of the specimen. It's suggested to measure the specimen using a jeweler's scale. If scalp hair isn't available, an equivalent amount of body hair may be collected. 'Head hair' denotes only scalp hair, while 'body hair' includes all other types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary processing of a sample into a laboratory's system. This process verifies the sample was properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic chain of custody system.

Screening provides an initial assessment for drug use. Although Screening is a cost-efficient method for ruling out drug use in most samples, a positive screening result requires confirmation to be legally valid. Presumptive positive samples detected during Screening must go through secondary confirmation.

Should a sample show presumptive positivity in the Screening phase, additional hair is taken from the initial sample for Extraction. During this stage, drugs are extracted from the hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), underscoring why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation, as necessary. The complete laboratory procedure, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
