What it is, How it works

When sourced near the scalp, hair can reveal an approximate 3-month detection span for drugs and alcohol. Hair collection is straightforward, somewhat resistant to tampering, and simple to transport.

A 1.5-inch portion of about 200 hairs (roughly the girth of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation. To test for EtG, additional substances, and/or beyond 10 panels, 150mg of hair is suggested. We advocate for measuring the hair sample with a jeweler’s scale. If scalp hair is missing, a comparable quantity of body hair may be obtained. When mentioning head hair, it exclusively means scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a sample into a laboratory's framework. This step includes ensuring that the sample was correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and executing any extra data entry absent from an electronic chain of custody system.

Screening performs an initial rapid check for abuse drugs. Although it is a cost-effective mechanism to exclude drug use in most samples, any positive result from Screening must be validated to be permissible in legal proceedings. Every sample showing a presumptive positive in Screening requires secondary confirmation.

Should the sample be presumptively positive in the Screening phase, more hair is taken from the initial sample for the Extraction phase. Here, drugs are leached out from hair at markedly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

For any positive result in screening, confirmation is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes washing before confirmation as necessary. The entire procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair standard and the accreditation to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
