What it is, How it works

When obtained near the scalp, hair can offer up to roughly a three-month detection period for alcohol and other substances. A segment of 1.5 inches comprising approximately 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimum quantity for evaluation and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the specimen with a jeweler’s scale. If scalp hair is missing, a comparable quantity of body hair may be collected. When referring to head hair, we pertain solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The process for laboratory examination of a drug test outcome consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary handling of a sample within a laboratory’s framework. This includes confirming that the sample was secured and transferred correctly, designating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody method.

Screening entails an initial rapid evaluation for substance abuse. While Screening offers a budget-friendly approach to exclude drug usage in most samples, a positive screen must be validated to be acceptable in legal proceedings. Samples that are presumptively positive during Screening necessitate a secondary confirmation.

In cases where a sample is tentatively positive during Screening, more hair is extracted from the initial specimen for Preparation. In this stage, substances are extracted from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug assessment is the most labor-intensive technique to execute.

Confirmation of any positive Screening finding is performed using GC/MS, GC/MS/MS, or LC/MS/MS methods. All tentative positive samples undergo cleaning prior to confirmation as necessary. The full laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
