What it is, How it works

Hair analysis has evolved into a significant method for identifying drug and alcohol consumption. Hair captures a prolonged record of substance use by embedding biomarkers into the fibers of the developing hair strand. When obtained near the scalp, hair can offer nearly a 3-month window for detecting alcohol and other substances. The collection of hair is straightforward, moderately hard to tamper with, and simple to transport.

A 1.5-inch portion of approximately 200 hair strands (about the diameter of a #2 pencil) close to the scalp yields 100mg of hair, which is the optimal sample size for evaluation and confirmation. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen using a jeweler’s scale. Should scalp hair not be available, a similar amount of body hair can be gathered. When discussing head hair, we specifically mean scalp hair. Body hair refers to any other hair type (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory handling of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into the laboratory’s system. This process includes confirming that the sample was appropriately sealed and shipped, assigning it a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. While Screening is an economical method to exclude drug use in most samples, a positive result necessitates confirmation for legal admissibility. Samples that are presumptively positive in Screening do require a secondary confirmation.

When a sample tests presumptively positive in the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, substances are extracted from hair at a much lower concentration compared to other techniques (e.g., urine or oral fluid), contributing to the complexity of hair drug screening.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair requirement and the compliance with ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
