A sample of 1.5 inches of approximately 200 hair strands (similar to a #2 pencil's girth) near the scalp will amount to 100mg of hair, the preferred sample size for screening and verification. For EtG, add-ons, and/or tests over 10 panels, a specimen of 150mg is suggested. We advise weighing the specimen on a jeweler’s scale. If scalp hair cannot be provided, an equivalent amount of body hair may be collected. When mentioning head hair, we are exclusively speaking of scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome includes four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial processing of a sample into a laboratory's system. This entails ensuring the sample's proper sealing and shipping, allotting a random LAN (Laboratory Accessioning Number), and completing any extra data input not provided by an electronic chain of custody system.

Screening encompasses an initial rapid check for drugs of abuse. While Screening is a budget-friendly method to exclude drug usage on the majority of specimens, a positive screen must be verified to be valid in judicial settings. Any specimens preliminarily positive in Screening demand a secondary confirmation.

If a specimen is preluded as positive in the Screening phase, additional hair is extracted from the initial specimen and prepped for Extraction. In this stage, drugs are withdrawn from hair at a substantially reduced concentration than in other methodologies (e.g., urine or oral fluid), making hair drug screening the most intricate method to undertake.

Verification of any positive screening outcome is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive specimen is cleansed prior to confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
