Hair analysis has come to be recognized as an effective method for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and drug intake by embedding biomarkers within the fibers of the growing hair strands. Hair collected near the scalp can offer up to around a 3-month detection period for alcohol and drugs. It is easy to collect, somewhat difficult to alter, and convenient for shipping.
A 1.5-inch section consisting of roughly 200 hair strands (approximately the size of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, add-ons, and/or tests above 10 panels, 150mg of sample is advisable. We suggest weighing the sample on a jeweler's scale. If scalp hair is not available, a comparable amount of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).
Process Overview
The four fundamental stages in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves initially processing a sample within a laboratory's system. This includes confirming that the sample was sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.
Screening involves an initial rapid check for drugs of abuse. While Screening provides a cost-effective approach to eliminate drug use on the majority of samples, a positive screen needs confirming to be acceptable in court. Any samples that preliminarily test positive during Screening require further confirmation.
If a sample tests presumptively positive in the Screening phase, additional hair is taken from the initial sample and is prepared for Extraction. In this stage, drugs are extracted from hair at a far lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most challenging methodology to execute.
Confirmation of any positive screening result is achieved by using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The complete laboratory process from Accessioning to Confirmation is reviewed following both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
