Process Overview

The four essential phases in laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab's system. This process includes ensuring the sample was correctly sealed and delivered, allotting a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data input absent from an electronic chain of custody system.

If a sample tests preliminarily positive in the Screening step, additional hair is extracted from the initial collection and prepped for Extraction. This phase isolates drugs from hair at considerably lower concentrations than other procedures (e.g., urine or oral fluid), which renders hair drug screening the most challenging procedure to execute.

Confirmation of any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes a wash before confirmation, as necessary. The complete laboratory operation from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
