A 1.5-inch segment comprising roughly 200 hairs (comparable to a #2 pencil in thickness) nearest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and confirmation purposes. For EtG additions and/or analyses exceeding 10 panels, it is recommended to gather 150mg of the sample. It is advisable to measure the sample on a jeweler’s scale. In situations where scalp hair is missing, an equivalent volume of body hair is acceptable. References to head hair are specifically about scalp hair, while body hair encompasses all other types (like facial or axillary hair).

Process Overview

The laboratory steps fundamental to processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of integrating a received sample into a lab's system. This step involves confirming that the sample has been properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data input not already provided by an electronic custody tracking system.

Screening comprises an initial rapid check for drug abuse. Though Screening is an economical approach to eliminate drug usage for most samples, a positive screen must be verified to be legally accepted in court. Any samples indicating potential positivity in Screening necessitate further confirmation.

Should a sample indicate potential positivity during Screening, additional hair is taken from the initial sample for Extraction. During Extraction, drugs are isolated from hair at much lower concentrations than in other methodologies (such as urine or oral fluid), making hair drug testing one of the most challenging methodologies to perform.

Confirmation of any preliminary positive screen is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All initial positive samples undergo cleansing before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair standard and complies with ISO / IEC 17025 accreditation.
