A 1.5-inch portion of around 200 hair strands (approximately the thickness of a #2 pencil) from the scalp will yield 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg amount is advised. We suggest using a jeweler’s scale to weigh the hair sample. In the absence of scalp hair, a comparable quantity of body hair can be used. Scalp hair specifically refers to the hair on the head, while body hair encompasses all other kinds (such as facial and axillary hair).

Process Overview

The laboratory processing of drug tests includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing within the laboratory's system. This step includes verifying the sample's proper sealing and shipping, assigning a randomized LAN (Laboratory Accessioning Number), and filling in any additional information not captured by an electronic chain of custody system.

Screening provides an initial quick assessment for commonly abused drugs. While Screening is a cost-effective strategy to exclude drug use in most samples, a positive result from this stage must be confirmed for legal admissibility. Any samples with presumptive positive results from Screening proceed to confirmation.

If Screening suggests a positive result, the original specimen is further sampled for Extraction. During Extraction, drugs are isolated from the hair at a concentration lower than those used in other methods (such as urine or oral fluid testing), highlighting the complexity inherent in hair drug screening.

Confirmation follows any positive Screening result using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptively positive samples are washed before confirmation, as necessary. The complete process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
