For EtG, add-ons, or tests exceeding 10 panels, a 150mg specimen is suggested. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair isn't accessible, an equivalent quantity of body hair can be gathered. When we mention head hair, we specifically refer to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into the laboratory system. This step includes ensuring the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid examination for drug abuse. While Screening is a cost-efficient means to eliminate drug use for most samples, any positive result must be confirmed for court admissibility. Presumptive positive samples in Screening require subsequent confirmation.

When a sample shows a presumptive positive in the Screening phase, more hair is removed from the initial sample and prepared for Extraction. At this stage, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening a challenging methodology.

Confirmation for any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a washing process before confirmation as necessary. The end-to-end laboratory process from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
