What it is, How it works

A 1.5-inch section of approximately 200 hair strands (equivalent to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for examination and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is suggested. We advocate for weighing the specimen on a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four principal steps in the laboratory handling of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s system. This consists of ensuring the sample was sealed and dispatched correctly, issuing a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not supplied by an electronic chain of custody system.

Screening is an initial brief assessment for drugs of abuse. While Screening is an economical way to eliminate drug usage in most samples, a positive screen must be validated for court admissibility. Any samples that test presumptively positive in Screening necessitate secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial specimen and prepped for Extraction. During this phase, drugs are extracted from hair at a much lower concentration than in other procedures (e.g., urine or oral fluid), which is why hair drug screening is the most challenging process to execute.

Verification of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positivity are cleansed before confirmation if necessary. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
