What it is, How it works

Hair analysis is now regarded as a significant method for spotting drug and alcohol consumption. Hair acts as a timeline for alcohol and various substances by embedding biomarkers in the hair strands as they grow. When taken from close to the scalp, hair can reveal alcohol and other drug usage for approximately up to a 3-month duration. Hair is straightforward to gather, relatively hard to tamper with, and straightforward to mail.

A 1.5-inch segment of around 200 hair strands (approximately the width of a #2 pencil) near the scalp provides 100mg of hair, which is the optimal quantity for assessment and verification. For EtG, additional tests, and/or panels above ten, it's advisable to use 150mg of hair. For accuracy, use a jeweler’s scale to weigh the hair. If scalp hair isn’t accessible, a similar volume of body hair can be taken. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four crucial phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into a lab’s system. It confirms that the sample was properly sealed and transported, allocates a unique LAN (Laboratory Accessioning Number), and fills in any extra data not captured by an electronic chain of custody system.

Screening provides a preliminary rapid check for drug use. Although Screening is a budget-friendly way to eliminate drug usage in most samples, a positive result requires confirmation for legal admissibility. Any preliminary positive samples in Screening necessitate a secondary confirmation.

If a sample shows a preliminary positive in the Screening phase, additional hair is taken from the original collection and prepped for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than other techniques (such as urine or oral fluid), making hair drug analysis the most challenging technique to execute.

Confirmation of any positive result in screening is done with GC/MS, GC/MS/MS, or LC/MS/MS. All samples with preliminary positives are cleaned before confirmation if required. The full lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair criteria and is certified according to ISO / IEC 17025 standards.
