Collected near the scalp, hair can reveal drug or alcohol use over a roughly 3-month period. The collection process for hair is straightforward, challenging to tamper with, and easy to transport.

A 1.5-inch segment of around 200 hair strands (comparable to the thickness of a #2 pencil) near the scalp provides 100mg, an optimal amount for screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg is advisable. We suggest using a jeweler’s scale for specimen measurement. If scalp hair is unavailable, equivalent body hair can be collected. By head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the sample's initial integration into a lab’s system. This process ensures the sample was sealed and dispatched correctly, allocates a random LAN (Laboratory Accessioning Number), and completes remaining data entry not covered by an electronic chain of custody system.

Screening is a preliminary quick evaluation for substance abuse. While it’s a cost-effective technique to exclude drug use for most samples, a confirmed positive screen is needed for legal proceedings. Any samples appearing positive in Screening require secondary confirmation.

If Screening suggests a positive result, additional hair is extracted from the primary specimen and readied for Extraction. During this phase, substances are isolated from hair at much lower concentrations than other methods (such as urine or oral fluid), which highlights the complexity of hair drug analysis.

Positive Screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed before confirmation as necessary. The laboratory workflow from Accessioning to Confirmation is scrutinized under CAP’s (College of American Pathologists) Hair criteria and accredited to ISO / IEC 17025 guidelines.
