Collecting a 1.5-inch section consisting of about 200 strands of hair (approximately the size of a #2 pencil) from near the scalp yields 100mg, which is ideal for screening and confirmation. For EtG, additional tests, or those exceeding 10 panels, 150mg is suggested. It's advisable to measure the sample using a jeweler’s scale. If scalp hair is inaccessible, a similar quantity of body hair can be used. By head hair, we refer specifically to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The procedure of processing drug test outcomes in the laboratory includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into a lab’s database. This phase consists of ensuring the sample’s seal and shipping were intact, assigning a LAN (Laboratory Accessioning Number) at random, and completing supplementary data entry not handled by an electronic chain of custody system.

Screening is an initial examination for drugs. Although Screening is a cost-effective technique to eliminate drug use in most cases, a positive result requires confirmation for court acceptance. Samples with presumptive positives in Screening necessitate further confirmation.

For samples with presumptive positives during Screening, more hair is extracted from the original sample and prepared for Extraction. In this step, lower concentration drug extraction occurs compared to other methods (ex. urine or oral fluid), hence hair drug analysis is the most challenging method to perform.

Confirmation of positives in screening is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if necessary. The complete procedure from Accessioning through Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation along with ISO / IEC 17025 standard accreditation.
