What it is, How it works

Hair testing is increasingly recognized as an effective means for identifying drug and alcohol consumption. Hair captures long-term drug and alcohol consumption histories by embedding biomarkers within the strands of growing hair. When gathered close to the scalp, hair can offer roughly a 3-month detection period for drugs and alcohol. Collecting hair is straightforward, is relatively resistant to tampering, and can be shipped without difficulty.

A 1.5-inch hair sample of about 200 strands (similar in size to a #2 pencil) nearest the scalp will yield 100mg of hair, which is optimal for screening and confirmation. For EtG, add-on tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, a comparable amount of body hair may be used. When we mention head hair, we mean scalp hair specifically, while body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into a laboratory’s system. This phase involves confirming the sample's secure sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening entails a rapid initial check for drugs of abuse. Although Screening is an economical approach to ruling out drug usage for the bulk of samples, a positive Screening result requires confirmation in order to be valid in legal contexts. Any samples that are presumptively positive during Screening require additional confirmation.

If a sample tests presumptively positive during the Screening stage, additional hair is removed from the initial specimen to prepare for Extraction. At this stage, drugs are extracted from hair at lower concentrations compared to other techniques (such as urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples undergo a wash prior to confirmation, as necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair indicators and the ISO / IEC 17025 standards accreditation.

Advantages of hair drug testing:

  • Prolonged detection period: Hair drug tests may detect drug use up to 90 days, unlike urine tests with a shorter detection window.
  • Challenging to falsify: Hair drug tests are difficult to deceive, enhancing result accuracy.
  • Offers historical insights: It reveals a drug use pattern over a timeframe, rather than only recent usage.

Limitations:

  • Inability to detect recent use: It requires about 5-7 days for drugs to become traceable in hair.
  • Expense: Hair drug tests tend to be costlier than other testing methods.
  • Result variability: Factors such as hair color and personal hair growth differences can influence drug metabolite concentrations in hair.

Note: Often called "hair follicle tests", the analysis is conducted on the hair strand itself, not the hair follicle under the scalp.

Why Hair Follicle Testing Beats Urine Testing

In Steens, MS, hair follicle testing outperforms traditional urine tests in terms of effectiveness and accuracy.
