What it is, How it works

Hair embeds a prolonged account of alcohol and other substances by capturing markers within the strands of growing hair. When gathered near the scalp, hair can offer up to a 3-month detection period for alcohol and other substances. Hair is easy to gather, somewhat tough to alter, and simple to send.

A 1.5-inch hair sample consisting of around 200 strands (roughly the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect sample for analysis and verification. For EtG, additional tests, or examinations beyond 10 panels, 150mg of the sample is advised. We recommend using a jeweler’s scale for weighing the specimen. If scalp hair is absent, a similar amount of body hair can be used. When we say head hair, we mean only scalp hair. Body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial input of a sample into a lab’s system. This consists of confirming the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary rapid check for addiction substances. Although Screening efficiently rules out drug usage on most samples, a positive screen must be validated to be accepted in legal processes. Any samples showing presumptive positivity in Screening need further validation.

If a sample indicates presumptive positivity in the Screening stage, additional hair is taken from the initial sample and prepared for Extraction. This phase involves drug extraction from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug analysis the most complex method to execute.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
