A 1.5-inch section of roughly 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for testing and validation. For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair cannot be obtained, a similar volume of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the initial integration of a sample into a lab’s system. This entails confirming that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not handled by an electronic chain of custody system.

Screening is a preliminary rapid test for drug abuse. Screening is an economical approach to exclude drug use in most samples; however, any positive screen must be verified to be court-admissible. Any samples showing a presumptive positive result in Screening require a secondary verification.

If a sample shows a presumptive positive result in Screening, additional hair is extracted from the original specimen and organized for Extraction. During this phase, drugs are retrieved from hair at concentrations much lower than in other processes (e.g., urine or oral fluid), hence making hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples undergo washing as required. The entire lab procedure from Accessioning to Confirmation is carried out under both the CAP (College of American Pathologists) Hair title and the accreditation for ISO / IEC 17025 standards.
