What it is, How it works

Hair analysis has emerged as a robust technique for identifying drug and alcohol consumption. Hair captures long-term evidence of alcohol and drugs by embedding biomarkers within its strands. When sampled near the scalp, hair can offer an approximate 3-month detection timeframe for substances. Collecting hair is straightforward, hard to tamper with, and simple to transport.

A 1.5-inch sample of roughly 200 hair strands (similar to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal amount for testing and validation. For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is advisable. It’s suggested to use a jeweler’s scale to weigh the sample. If scalp hair isn't available, a comparable quantity of body hair can be used. When mentioning head hair, we specifically refer to scalp hair. Body hair includes all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory process for drug testing includes four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where the sample is registered into the lab's system. This process verifies the sample’s proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry that an electronic chain of custody system might not provide.

Screening is the preliminary and rapid evaluation for drug presence. While it’s a cost-efficient method to identify potential drug use in most samples, a positive result here must be validated to be court-admissible. Samples marked presumptively positive in the Screening phase require further confirmation.

Upon a presumptive positive result during Screening, additional hair is extracted from the initial sample and readied for Extraction. This stage involves extracting drugs from hair at far lower concentrations than other techniques (e.g., urine or saliva), making hair drug testing one of the most challenging methods to conduct.

Positive screening confirmations are completed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive result are cleansed before confirmation as necessary. From Accessioning to Confirmation, the entire process is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Note: Although frequently termed as "hair follicle tests", the analysis targets the hair strand, not the follicle beneath the scalp.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
