A sample of hair measuring 1.5 inches, consisting of about 200 strands (approximately the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the preferred quantity for both screening and confirmation. For EtG additions or tests exceeding 10 panels, a specimen weighing 150mg is advised. The specimen should be weighed using a jeweler’s scale. If scalp hair cannot be obtained, a similar amount of body hair can be gathered. References to head hair specifically mean scalp hair. The term body hair encompasses all other hair types (such as facial or axillary hair).

Process Overview

The four primary stages in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where a sample is introduced into a lab’s system. This step ensures that the sample was properly sealed and shipped, assigns it a random Laboratory Accessioning Number (LAN), and completes any additional data entry required not covered by an electronic chain of custody system.

The Screening phase conducts a preliminary assessment for the presence of drugs. Although Screening offers a cost-effective approach to exclude drug usage in most samples, any positive result must be confirmed for it to be legally admissible. Samples that are tentatively positive in the Screening stage need a subsequent confirmation.

If a sample tests presumptively positive during Screening, further hair is extracted from the initial sample and readied for Extraction. This phase involves extracting drugs from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), which is why hair drug testing is considered the most challenging methodology to execute.

Confirmation of any positive finding from the screening phase is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with preliminary positive results are cleansed prior to confirmation when required.
