A sample measuring 1.5 inches with around 200 hair strands (akin to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and verification. For tests involving EtG, additional markers, and/or over 10 panels, 150mg of the sample is advised. It's recommended to weigh the sample using a jeweler’s scale. If scalp hair is not obtainable, a comparable quantity of body hair can be used. When mentioning head hair, it pertains exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four pivotal steps in the lab handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the starting phase of processing a sample in a lab’s system. It involves checking that the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any supplementary data entry not furnished by an electronic chain of custody system.

Screening consists of an initial rapid check for drugs of abuse. Although Screening is an economical method to exclude drug use in the majority of samples, a positive screen must be validated to be regarded as valid in court. Samples marked presumptive positive in Screening require an additional confirmation.

If a sample is found presumptive positive during the Screening phase, additional hair is extracted from the initial sample and prepped for Extraction. In this phase, drugs are separated from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is considered the most complex method to execute.

Any positive screening outcome is validated using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleaned before confirmation as necessary. The whole lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair directive and the accreditation to ISO / IEC 17025 benchmarks.
