What it is, How it works

When procured near the scalp, hair can offer roughly up to a 3-month detection timeframe for alcohol and other drugs. It is easy to collect, somewhat resistant to tampering, and simple to ship.

A 1.5-inch section consisting of roughly 200 hair strands (resembling the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for analysis and verification. For EtG, add-ons, and/or testing beyond 10 panels, 150mg of the sample is advisable. We suggest using a jeweler's scale for weighing the sample. If head hair is not available, an equivalent quantity of body hair can be collected. When we mention head hair, it specifically refers to scalp hair. Body hair includes all other hair types (facial, underarm, etc.).

Process Overview

The four principal stages in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab’s system. This incorporates confirming that the sample was correctly sealed and dispatched, generating a random LAN (Laboratory Accessioning Number), and finishing any additional data entry not already provided by an electronic chain of custody system.

Screening comprises an initial rapid evaluation for drugs of misuse. Although Screening is a cost-efficient approach to dismiss drug usage in most samples, a positive screening must be validated to be acceptable in court. Samples that are presumptively positive during Screening need secondary confirmation.

Should a sample read presumptive positive at the Screening stage, more hair is extracted from the initial specimen and readied for Extraction. At this phase, drugs are withdrawn from hair at a notably lesser concentration than in alternative methodologies (such as urine or oral fluid), which is part of why hair drug screening is the most complex methodology to conduct.

Verification of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. Every presumptive positive sample receives a wash before verification as necessary. From Accessioning to Confirmation, the entire laboratory process is scrutinized under the CAP (College of American Pathologists) Hair designation as well as accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
