What it is, How it works

A sample measuring 1.5 inches, consisting of roughly 200 strands (similar to a #2 pencil's thickness) near the scalp provides 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, or those exceeding 10 panels, 150mg of the sample is advised. It's recommended to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, a comparable amount of body hair can be used. "Head hair" refers solely to scalp hair, whereas "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four key stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes preliminary processing into the lab’s system. This involves verifying that the sample is sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input absent from an electronic chain of custody system.

Screening includes a swift initial assessment for drugs of abuse. Although Screening is cost-efficient for excluding drug usage in most samples, a positive screen must be verified to be valid in legal proceedings. Samples that test presumptive positive in Screening require additional verification.

If a sample tests presumptive positive at the Screening phase, more hair is extracted from the original sample for Extraction. At this point, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), underscoring why hair drug tests are the most challenging methodology.

Any positive screening is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire lab procedure, from Accessioning to Confirmation, is evaluated according to the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
