What it is, How it works

Hair collection is straightforward, harder to alter, and convenient to transport.

A 1.5-inch portion of about 200 hair strands (akin to a #2 pencil's thickness) near the scalp yields 100mg, ideal for analyzing. For tests exceeding 10 panels, EtG, or additional assessments, 150mg of the sample is suggested. We advise using a jeweler's scale for weighing. If head hair is unavailable, body hair can be used as a substitute. When we talk about head hair, it specifically refers to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result includes four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the introductory processing of a specimen within a lab’s framework. This verifies that the sample is correctly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes necessary data entry if an electronic chain of custody system doesn't provide it.

Screening involves an initial prompt inspection for drug traces. While it is a budget-friendly method to exclude drug presence in most samples, a positive result must be verified for court acceptance. Samples showing preliminary positives in Screening need a subsequent confirmation.

If a sample appears positive during Screening, additional hair is extracted from the sample for Extraction. Here, drugs are isolated from hair, presenting a greater challenge compared to other techniques (e.g., urine or oral fluid), highlighting hair drug screening as the most challenging methodology.

Confirmation of any positive screening outcome uses GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives undergo washing before confirmation if required. The complete lab process from Accessioning to Confirmation undergoes review under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
