What it is, How it works

Hair analysis has been recognized as an effective method for identifying drug and alcohol consumption. When taken near the scalp, hair can offer nearly a 3-month period for detecting alcohol and other drugs. Hair collection is straightforward, is relatively resistant to tampering, and can be easily transported.

Collecting a 1.5-inch segment of roughly 200 hair strands (comparable to the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the perfect amount for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is suggested. It is advised to measure the sample with a jeweler’s scale. Should scalp hair not be available, a similar amount of body hair can be collected. Head hair refers specifically to scalp hair. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This involves ensuring the sample was sealed and delivered correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.

Screening entails a preliminary assessment for drugs of abuse. Although Screening is an economical approach to eliminating drug use for most samples, any positive result requires confirmation for it to be legally admissible. Any samples that test presumptively positive during Screening must undergo secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is taken from the original specimen and prepared for Extraction. During this phase, drugs are removed from hair at significantly lower concentrations than in other methods (such as urine or oral fluid), highlighting why hair drug screening is the most challenging methodology to conduct.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are cleansed as necessary. The complete laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
