In the absence of scalp hair, an equivalent quantity of body hair may be utilized. When discussing head hair, we specifically mean scalp hair. Body hair encompasses other types, such as facial and axillary hair.

Process Overview

There are four primary steps in the laboratory process for producing a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary stage where a sample is incorporated into the lab’s system. This step includes confirming the sample’s proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry that an electronic chain of custody system doesn’t cover.

The Screening stage offers a rapid initial assessment for the presence of drugs. It serves as an efficient means to exclude drug use in most samples, though a positive result during Screening necessitates confirmation for legal acceptance. Samples with presumptive positive results from Screening require a follow-up confirmation.

Should the Screening suggest a positive result, additional hair is taken from the primary specimen and prepped for the Extraction phase. Here, drugs are isolated from hair at lower concentrations compared to other methods, such as urine or oral fluid, rendering hair drug screening the most complex to execute.

Confirmation of any initial positive findings is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptively positive samples are rinsed before confirmation, as needed. The full laboratory sequence from Accessioning to Confirmation undergoes scrutiny under the CAP (College of American Pathologists) Hair designation and follows ISO / IEC 17025 accreditation guidelines.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
