What it is, How it works

Hair analysis has emerged as a robust method for identifying the use of drugs and alcohol. Hair captures a lasting record of alcohol and other substances by embedding biomarkers within the strands of the growing hair. When sampled near the scalp, hair can reveal up to roughly a 3-month detection period for alcohol and drugs. Hair is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch section of around 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample size for analysis and verification. For EtG, additional examinations, and/or panels above 10, a 150mg sample is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair is not an option, an equivalent quantity of body hair can be used. When we mention head hair, it specifically means scalp hair. Body hair refers to all other categories (facial, axillary, etc.).

Process Overview

The core phases in the laboratory workflow for processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's system. This process verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not provided by an electronic chain of custody system.

Screening involves a preliminary assessment for substances of abuse. While Screening is a cost-efficient way to eliminate drug presence in most samples, a positive result must be confirmed for legal admissibility. Any samples that show a presumptive positive in Screening need further confirmation.

If a sample shows a presumptive positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. At this stage, drugs are isolated from hair at significantly lower concentrations than in other methods (e.g., urine or saliva), which is why hair drug screening is the most complex method to execute.

Verification of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
